Chinese Medicine intake form
If you are seeing a Chinese medicine practitioner at Watershed, this is the form you need to fill out – this is particularly meant for Eric Grey’s patients. Discuss with your practitioner to find out which form is best for you. Fill it out to the best of your ability – leaving blank any information you do not have. You can either bring it with you to your appointment, or fill it out, scan and email it to your practitioner.
Naturopathic health history form
Use this form if you are coming to see Dr. Skye Nehs ND. Fill it out and bring it to your appointment, or scan and email it back to him at skye@watershedcommunitywellness.com.
Insurance benefit self-check form
If you want us to bill your insurance, we ask that you use this form to check the details of your benefits first. That way, there are no surprises as far as this aspect of your care is concerned. Please contact your practitioner if you have any questions about this form.
Basic Insurance Information needed
This form is what you will send to us if you want us to bill your insurance. If you provide us with this information, we can check on your coverage and verify that we are going to be able to bill for you.

This form lists Watershed’s basic clinic policies and will need to be signed before your first appointment.
Privacy Policies Acknowledgement form
This is the form that you sign after you have read the “Watershed Privacy Policies (HIPPA) information form”
Watershed Privacy Policies declaration
You will need to read through the information on this form, and then sign the “Privacy Policies Acknowledgment” form. These are the standard forms you fill out at most medical practitioners’ offices.
